Prescription Refill Request Form
Please read the instructions before submitting your request.
The form below allows you to request refills for your medication(s) online. During business hours, please allow 2-4 hours once the request has been submitted. If the request is submitted after hours, please allow 2-4 hours after we open the following business day. Please keep in mind that if approval from a Doctor is required or your prescription needs to be compounded, your request may take up to 24 hours. If you would like it delivered (within Idaho Falls and Ammon city limits only), there will be a charge of $3.00 and the prescription will be delivered after 5 pm. If you live outside Idaho Falls and Ammon city limits and would like your prescription mailed, there will be an additional charge around $6.00 depending on the weight and size of the prescription. Any prescription delivery or mail request submitted after 4 pm will be delivered or mailed the following day.
